I don't know about most other states, but as an election judge in
Minnesota, I can affirm that it is possible to register to vote on
election day in Minnesota (one doesn't actually need ID to do so,
although ID makes the whole process easier).

In Minnesota, any election observing by other than a certified
challenger would have to be done at least 100 feet away from the
polling location. And I'm pretty sure one would have to be a
citizen to get certified as a challenger.
